A list of available documents is found to the left. They are grouped according to the document format. Use whichever format you prefer.

To register for the swim team, you must submit three (3) forms:

1. The Registration Form, a two-page document that contains the fee schedule and your family contact and medical information. This year (2009) you can put all your children on one form.
2. The Liability Form.
3. The Behavioral Policy, a two-part document. Section A is for swimmers and the Section B is for parents.

You are required to sell a minimum of 20 subs or buy out of the sale for $30.00.
Sales are cumulative, so if you participate in 2 sales by selling 10 subs each time,
you will make your 20 sub minimum.
